Evelyn Forman (Lutsky)

Evelyn G. Forman, 100 years old, passed away peacefully on September 12, 2025. Trained as a Registered Nurse and married to Melvin Goldberg, she later earned a Doctorate in Epidemiology from Johns Hopkins University, where she served for years as an Associate Professor. After Melvin's death in 1988, she married Len Forman in 1995.

She is survived by her sons Jed, Sam, and Arthur Goldberg, as well as Grandchildren Lindsay Tateishi, Nicole Miller, Natalie Goldberg Jenner, Adam Goldberg, Emily Wynsma, Allison Goldberg Ferreira; and Great Grandchildren, Michael, Joey, and Laura Tateish; Matthew, Bekah, Nathan, and Joanna Wynsma; Leo and Charles Jenner; and Benjamin Goldberg.
